There are three ways to submit an application to Purnell.

Download the Transcript Request Form to be completed by the parent/guardian and given to the applicant’s current school.
Note: Educational Testing results should be mailed, emailed or faxed to the Admissions Office
Download the teacher recommendation forms. After you have filled out your name and address on the top of the form, please give it to your current teacher or school administrator to complete. They should return the form DIRECTLY to Purnell School electronically (scanned), by fax, or by U.S. Mail. Please note, when sending things by U.S. mail you MUST include P.O. Box 500.
English Teacher Recommendation
To be completed by the applicant's current English Teacher
Math Teacher Recommendation
To be completed by the applicant's current Math Teacher
To be completed by the applicant's Guidance Counselor, principal, coach, tutor, etc.
